Pepto Bismol, the pink US stomach remedy, is hard to buy in France. Its active ingredient, bismuth subsalicylate, belongs to the bismuth family of medicines, which French regulators have treated with deep caution for about 50 years.

The caution goes back to the 1970s, when France recorded an outbreak of bismuth encephalopathy, a rare brain toxicity that caused confusion, tremors and muscle jerks. Most cases were linked to other bismuth salts, such as subnitrate and subgallate, and patients generally recovered after stopping the drug. Even so, the whole bismuth family has carried that reputation ever since.

A second concern is salicylate, the aspirin-like part of the formula, which is linked to Reye’s syndrome in children. Today, France’s medicines agency still asks for extra monitoring when bismuth drugs are approved.

The situation is not the same across Europe. The UK brought Pepto-Bismol back in 2025 for ages 16 and over, while travelers report it is hard to find in Germany, Italy and Switzerland. In France, pharmacists usually suggest loperamide for diarrhea or an antacid for heartburn instead.

For the full background, read our explainer on why Pepto Bismol is so hard to find in Europe. France is not the only strict country on medicines either: Japan turns away Vicks inhalers and some cold medicines at the border.
